Temporary PORT Case Manager

Sanctum Care Group — Prenatal Outreach Resource Team (PORT)

Sanctum Care Group is seeking dedicated and passionate individuals to join our Prenatal Outreach Resource Team (PORT). This team delivers comprehensive, wrap-around support services to expectant women in Regina who are at risk of having their baby apprehended. PORT’s goal is to enhance health, wellness, stability, and parenting readiness through collaborative, community-based engagement.

Working closely with community partners, PORT Case Managers will support expectant mothers by coordinating psychosocial, mental health, addictions, and physical health services. Team members will address barriers as they arise and maintain a strong, consistent focus on the wellbeing of both the women and their unborn babies.

We are looking for excellent problem solvers, strong advocates, compassionate communicators, and adaptable team players. This work is highly community-based and requires comfort with fieldwork and independent decision-making. A vehicle is required for transporting clients.

Key Responsibilities

  • Provide case management support to prenatal and postnatal women in the community
  • Maintain accurate, timely charting and documentation
  • Work independently as well as collaboratively within the PORT team
  • Proactively engage clients within community settings
  • Work flexible field-based hours
  • Collaborate with community agencies and coordinate referrals as needed
  • Advocate for clients and demonstrate strong problem-solving skills in a fast-paced environment

Qualifications

  • Bachelor’s degree in Social Work, Human Services, or a related field
    • A combination of education and relevant experience will also be considered
  • Valid driver’s license
  • Reliable access to a vehicle
