Community Outreach and RESPONSE Team (cort)
SCG’s Community Outreach and Response Team aims to seek and treat community clients through a rapid and intensive model of care delivery. CORT aims to achieve viral suppression of HIV, improve patient + health outcomes, address barriers to access equitable and appropriate care, and establish support for long term medication management.
THIS PROJECT WILL:
- Increase viral suppression

why cort is needed
cort aims to:
- Initiate ARV treatment for patients who are at risk of adverse health outcomes due to lack of engagement in care for their HIV.
- Will improve the health and social wellbeing of the patients we serve.
- Will demonstrate the impact and importance of seek and treat models in patients who use drugs.
- Will decrease community viral load and AIDS related death in Saskatoon.
how we do it
- Target patients who need access to care that will allow for rapid treatment starts.
- Implement a team that will target patients who have CD4 counts less then 200 and are not on ARV’s.
- Quick, responsive, and reactive care
- Enhance patient engagement
- Coordinate care and facilitate access to support and care that removes barriers and increases engagement.
- Advocate for and support clients for equitable and appropriate care.
- Initiate ARV treatment for patients who are at risk of adverse health outcomes due to lack of engagement in care for their HIV.
