Community Outreach and RESPONSE Team (cort)

SCG’s Community Outreach and Response Team aims to seek and treat community clients through a rapid and intensive model of care delivery. CORT aims to achieve viral suppression of HIV, improve patient + health outcomes, address barriers to access equitable and appropriate care, and establish support for long term medication management.

why cort is needed

Sanctum Care Group works in partnership with Westside Community Clinic which is a primary care clinic set in the urban core neighbourhood of Saskatoon. Many active patients living with HIV are not currently on medications and do not have viral suppression, viral suppression allows for a normal healthy life expectancy. Due to the complexity of this population, there are many barriers that need to be addressed in order to achieve viral suppression of HIV. In partnership with Westside Community Clinic, CORT will provide an intensive case management approach that focuses on rapid medication starts and engagement in care. CORT is a dedicated strategy to improving medical and psychosocial engagement in this population.

SCG’s model of care incorporates harm reduction and trauma informed care principles that allows for low barrier, high threshold interventions rooted in patient centered care. CORT offers intensive case management interventions that focus on rapid anti retroviral treatment initiation while simultaneously addressing the psycho-social issues that prevent positive patient outcomes.
