Community Outreach and RESPONSE Team (cort)

SCG’s Community Outreach and Response Team aims to seek and treat community clients through a rapid and intensive model of care delivery. CORT aims to achieve viral suppression of HIV, improve patient + health outcomes, address barriers to access equitable and appropriate care, and establish support for long term medication management.

Increase viral suppression

why cort is needed

Sanctum Care Group works in partnership with Westside Community Clinic which is a primary care clinic set in the urban core neighbourhood of Saskatoon. Many active patients living with HIV are not currently on medications and do not have viral suppression, viral suppression allows for a normal healthy life expectancy. Due to the complexity of this population, there are many barriers that need to be addressed in order to achieve viral suppression of HIV. In partnership with Westside Community Clinic, CORT will provide an intensive case management approach that focuses on rapid medication starts and engagement in care. CORT is a dedicated strategy to improving medical and psychosocial engagement in this population.

cort aims to:

Initiate ARV treatment for patients who are at risk of adverse health outcomes due to lack of engagement in care for their HIV.
Will improve the health and social wellbeing of the patients we serve.
Will demonstrate the impact and importance of seek and treat models in patients who use drugs.
Will decrease community viral load and AIDS related death in Saskatoon.

how we do it

SCG’s model of care incorporates harm reduction and trauma informed care principles that allows for low barrier, high threshold interventions rooted in patient centered care. CORT offers intensive case management interventions that focus on rapid anti retroviral treatment initiation while simultaneously addressing the psycho-social issues that prevent positive patient outcomes.

Target patients who need access to care that will allow for rapid treatment starts.
Implement a team that will target patients who have CD4 counts less then 200 and are not on ARV’s.
Quick, responsive, and reactive care
Enhance patient engagement
Coordinate care and facilitate access to support and care that removes barriers and increases engagement.

How much does it cost?

It does not cost anything to receive services from the CORT Team.

What if I am homeless?

One of our goals is to help secure housing for the clients we work with.

What if I am still using drugs?

Our program is geared toward individuals struggling with substance use. This is not a barrier for accessing our services.

What if I don’t want all of the services provided?

We provide patient centered care, which means the client can choose which services are to be provided within our mandate.

Can I participate if I have a criminal record?

Yes, a criminal record is not a barrier to accessing services with the CORT team.

What if I don’t have a way for you to contact me?

Our team is very experienced with community outreach and will work with you to create a creative solution that works for you.
