Sanctum Care Group is recruiting its first Prenatal Outreach Resource Team (PORT) who will begin offering a comprehensive service in Saskatoon. Team members will case manage and provide a wrap around wide range of services for expectant women in Saskatoon who are at risk of their baby being apprehended. PORT will support these women to have a healthy pregnancy and successfully parent if they choose. Working closely with community partners the dynamic team members will work with the expectant moms to facilitate psycho social, mental health, addictions and physical care and services. They will be ready to tackle the barriers that present themselves and remain focused on the women and their unborn babies.
Successful applicants have a passion for embracing and delivering trauma informed care, harm reduction principles and meeting each person where they are at in their life journey. Ensuring all interactions are dignified, non judgmental, unconditional and without prejudice. Living the mission and values of Sanctum Care Group. An extensive knowledge of the community and diversity is critical. They work very well in teams and endeavor to fully engage the women and their supports.
They are excellent problem solvers, advocates, change agents, team players and interpersonal communicators. The work is carried out by actively engaging the women in the community. A vehicle is required.
Key Responsibilities
- Provide case management to pre and postnatal women the community
- Keep up to date charting and documentation
- Work independently and in a team setting
- Actively engaging clients in the community
- Ability to work field hours
- Connect with community agencies and provide referrals as needed
- Ability to advocate, problem solve and work in a face pace environment.
Successful candidate will have:
- Bachelor’s in social work, human services related field, and or combination of education and work related experience.
- Access to a vehicle is required.
- Validate drivers licence.
